Partnership in Transitional Care Workshop offered in French in Bourget: March 28th

Are you or someone you know considering a future move to long-term care? Partnership in Transitional Care is a free learning series available to those planning on making such a move within the next 2 years.

Alzheimer Society of Cornwall & District will offer this workshop in French at the Centre de santé communautaire de l’Estrie in Bourget on Wednesday, March 28^th 2012, from 6:00 to 7:30 p.m.).

• Reflection on your own personal circumstance

Preparing for the Move

• Common feelings at this time & ways to cope
• Planning for the day of the move
• Preparing your family member for the move
• Working with the Home and staff to provide the best possible care
• Helping your family member adjust to their new home
• Helping you adjust to your new role
