What is community health?

Community health is an approach that aims to empower communities. It relies on community members’ commitment to identify their needs and priorities and on implementing local initiatives.

The CSCE community health team

The team is made up of community health workers, health promoters, and health system navigators who encourage people to adopt healthy lifestyles.

They engage in activities such as:

  • helping people work together on committees to solve problems in their community;
  • helping people organize activities such as recreational activities for children, families and seniors;
  • offering educational activities on healthy lifestyles and ways to prevent chronic diseases and live better with them;
  • encouraging people to be physically active and learn how to eat well;
  • helping people navigate the health care system.

Support for navigating social, community, and health services

Health care navigators help people who have an active medical file at the CSCE find and access the services they need.

Eligible individuals:

  • A person who has at least one chronic disease and one or more social problems that have a major impact on their health (e.g. isolation, very low income, difficulty reading).
  • A person who has physical, cognitive, emotional, or social problems that prevent them from successfully obtaining the service they need. For example, a person may have trouble:
    • reading and writing;
    • making themselves understood when explaining their problem;
    • understanding information communicated by a professional;
    • staying calm when stressed because their problem is complicated;
    • using a computer or tablet to request a service through a website.

  • Services in English and French (Cornwall location offers services in French only).
  • In-person, telephone, or videoconference appointments.

  • Services offered to CSCE clients.

  • A referral must be made by your health care provider.

Green Food Box

The CSCE is a proud partner of the Green Food Box, a non-profit program that brings people in a community together to purchase a variety of quality fruits and vegetables at wholesale prices.

A Green Food Box costs $20 and contains high-quality fresh fruits and vegetables that are in season.

The CSCE’s Green Food Box program is available in Bourget, Crysler, Embrun, Finch, Limoges, Moose Creek, and Plantagenet.
