Diabetes Education Program
The Diabetes Education Program provides services to individuals aged 18 and older who have been diagnosed with prediabetes or type 1 or type 2 diabetes.
The team is made up of nurses and dietitians, most of whom are recognized by the Canadian Diabetes Educator Certification Board. The staff will consider your medical history, culture, lifestyle, preferences, and goals to provide you with personalized advice on:
- Blood sugar (glycemia)
- Preventing complications
- Treatment options (healthy lifestyle, medication, insulin)
General information
Services available in French and English. Individual consultations and group sessions. Appointments by phone, videoconference, or in person in Alexandria, Bourget, Cornwall, Crysler and Embrun.
Eligibility
Services are available to CSCE clients and community residents.

For health professionals
To refer a client to our services, fax the Diabetes Education Program – Referral Form or send us the following information:
- Service required (diabetes/prediabetes education, insulin initiation*, insulin adjustment*)
* Prescription and medical order required
- Preferred language for services
- Urgency of service (e.g. HbA1c > 10%, hypoglycemia)
- Name, contact information, and OHIP card number
- Diagnosis (prediabetes, type 1 diabetes, or type 2 diabetes)
- Recent lab test results (A1c, FPG, A1C, lipids)
- Medical history
- Up-to-date medication list

Diabetes: The Essentials
This session will teach you about the effects of lifestyle on diabetes and prediabetes. You will learn strategies for controlling blood sugar and preventing complications.
- Duration: 60 to 90 minutes
- Format: virtual or in person (in person in Cornwall and Bourget only)
- Language: French or English
