Primary Care
Primary care is the care you receive from your family doctor or nurse practitioner in collaboration with nurses.
These are the people who monitor you closely; they receive all your test results and are familiar with your health status.
They can:
- help you stay healthy.
- perform exams;
- ask questions to understand your symptoms;
- find out what is wrong;

Our teamwork approach promotes collaboration between the various health professionals at the CSCE who can offer you various services to help you achieve your health goals or manage certain illnesses. These individuals can offer advice on disease prevention and help you better understand your health and how to take care of yourself.
Our doctors and nurse practitioners will refer you to outside specialists as well as to programs and services offered in the community where you will find various forms of support and expertise.

Types of appointments
Depending on your needs and the reason for your appointment, the office can offer you appointments:
- In person;
- By phone;
- By videoconference.
It is important to note that, depending on your medical condition or symptoms, an in-person appointment may be required.

- Make an appointment online
- Call the location where you receive your care during business hours
Cancelling appointments
You must notify us as soon as possible if you need to miss an appointment so that we can offer it to someone else.
Emergency appointments
Emergency appointments are available every day. If your doctor or nurse practitioner is unable to see you, you may be offered an emergency appointment with another professional.
Transcription software
Our nurse practitioners and physicians use AI Scribe software. It prepares a clinical note by recording the appointment, transcribing it, and organizing it in a logical and useful way. At the end of the appointment, the software produces a clinical note that the physician or nurse practitioner reviews and edits as needed. Personal and confidential data is then deleted or anonymized.
This software allows your health care provider to have a better relationship with you during the appointment. It also allows them to spend less time on administrative tasks.
Access to care outside of normal business hours
The East Region Virtual Care Clinic provides free care for a variety of health issues. This clinic is open 7 days a week, from 1:00 PM to 9:00 PM. Please visit the website to confirm whether the clinic can help you with your health issues.

This service is available to everyone living in Eastern Ontario, regardless of whether they have a family doctor.

Remote Care Monitoring
The Remote Care Monitoring Program is offered in partnership with the St. Joseph’s Continuing Care Centre. This program is available to people who have been discharged from the hospital and who could benefit from having their health monitored at home. It provides:
- Loan of easy-to-use medical equipment
- Follow-up phone calls with a nurse
The goal of this support is to prevent rehospitalization or deterioration of health.

To access services
A referral must be made by your doctor or nurse practitioner.
Vaccination
Vaccination is the best way to protect yourself against certain viruses, such as influenza. Every fall, the CSCE works with the Eastern Ontario Health Unit to organize vaccination clinics. Vaccines against influenza, respiratory syncytial virus (RSV), and other viruses are offered.
If you have any questions about vaccines, talk to your doctor or nurse practitioner.
Healthy Baby
This service allows a nurse practitioner to monitor your child’s health. Your baby will be seen 8 times during their first two years, according to the recommendations for newborn care.
During these visits, the following will be checked:
- Coping skills.
- Language skills;
- Motor skills (movements);
- Development;
A dietitian can meet with you when your baby starts eating solid foods. A nurse practitioner can also help you with breastfeeding by providing information and advice and, if necessary, recommending treatment.

You can register your baby by calling the CSCE nearest you.
Memory Clinic
The Memory Clinic team provides personalized assessment and follow-up for people with mild cognitive impairment and dementia, in collaboration with caregivers. Referrals to community services are also made as needed. Care is provided based on the model of the MINT Memory Clinic, founded by Dr. Linda Lee.
The team consists of a physician, nurses, a health system navigator, and a family support services counselor from the Alzheimer Society of Cornwall and District.
Services
The services include cognitive assessment and testing, an interview with a family member, and a team consultation with the client and their family member.
